What you'll do

• Screen medically complex children entering ChildNet’s care and guide intake and placement decisions
• help intake staff obtain medical histories
• assist caregivers with identifying specialists, scheduling appointments, and following physician recommendations
• provide medical care training and support to foster parents, including help obtaining equipment and supplies
• liaise between medical professionals and caregivers
• audit case files quarterly
• submit CMS referrals
• maintain accurate records of children with medical diagnoses.
