What you'll do

• Manage incoming referrals and admissions
• coordinate hospital and healthcare-setting transitions
• review referral documentation and pre-admission requirements
• verify insurance coverage, benefits, and payer requirements
• use PointClickCare to manage admissions information and workflows
• maintain records and follow up on referrals
• communicate decisions and next steps
• collaborate with facility leadership and clinical teams
• maintain referral relationships and support census growth through referral development and marketing activities as needed.
