What you'll do
- Screen and assess clients facing housing displacement or unsheltered situations
- identify underlying issues and possible solutions
- maintain a small short-term caseload when needed
- provide information and referrals
- triage and track referrals and shelter turnaways
- coordinate with community agencies
- enter assessments in HMIS and Salesforce
- complete progress notes within 48 hours.
