The Registered Nurse (RN), Home Health Admissions role is an onsite, territory-based position serving the Twin Cities Metro Area from the Jordan, MN listing location. The nurse leads the start of each home health episode by assessing patients, documenting OASIS information, and establishing an individualized care plan. This first clinical visit helps patients and families understand services and supports safe transitions into home health.

What you'll do

• Complete Start of Care admissions
• perform nursing assessments and OASIS documentation
• develop individualized plans of care
• educate patients and families about diagnoses, medications, services, and expectations
• coordinate with physicians, case managers, therapists, and the interdisciplinary team
• support transitions from hospitals, skilled nursing facilities, and clinics.
