What you'll do

• Coordinate immediate care needs for youth hospitalized with behavioral health conditions and enrolled in OhioRISE
• support discharge planning, educate families and community partners, and refer youth for follow-up services. Complete engagement milestones including safety plans, assessments, care plans, and Child and Family team meetings
• coordinate family-based services and community partnerships, document work, and participate in training and program planning.
