What you'll do

• Assist clients with behavioral and personal care services and activities of daily living according to care plans
• accompany clients on recreation and leisure activities
• implement Behavior Support Plans and Individual Service Plans
• track and chart data daily
• attend and participate in team meetings as needed
• promote client independence and community involvement.
