HCC Practice Performance Manager

The HCC Practice Performance Manager role is onsite in Wichita, KS, with frequent local travel to physician practices throughout the market. Pay is $80,000–$95,000 per year. The manager works with physicians and practice leaders to assess performance, improve documentation and HCC capture, and implement measurable improvement plans. What you'll do

• Develop relationships with physicians, clinic managers, and practice leaders
• improve HCC capture, clinical documentation, coding accuracy, and risk adjustment performance
• review practice data and identify trends and gaps
• conduct onsite provider meetings and establish improvement plans
• educate providers on documentation
• monitor HCC, quality, and value-based care performance
• maintain communication and follow-up with practices
• collaborate on population health and primary care initiatives
• manage provider meetings, reporting, travel, and follow-up independently.
