What you'll do

• Coordinate and monitor psychiatric rehabilitation services for an assigned caseload
• orient residents to the facility and treatment plans
• conduct assessments and facilitate care-plan conferences
• provide or coordinate individual counseling, groups, training, and behavioral interventions
• document progress and resident responses
• coordinate with interdisciplinary teams, families, staff, and community agencies
• support referrals and discharge planning.
