Community Liaison

Job overview

The Community Liaison position is onsite in Akron, OH, with annual compensation of $75,000–$90,000. The role supports Crossbridge Hospice’s mission to provide compassionate, patient-centered hospice care that honors each person’s dignity. The liaison connects hospice services with the local community, developing referral relationships and supporting business development. Through outreach, education, timely referral follow-up, and coordination with clinical teams, the role helps patients and families understand hospice services and supports access to care.

What you'll do

• Identify and coordinate healthcare-community in-services
• build relationships with hospitals, physician offices, senior living communities, and other referral sources
• pursue opportunities to grow patient census
• represent the organization at community events and professional networking functions
• educate patients, families, caregivers, and legal representatives about hospice services and enrollment
• coordinate communication between referral sources and clinical teams
• follow up on referrals and document outreach
• provide education to healthcare and community staff
• analyze referral patterns and monitor market trends and competitor activity.
