• Perform in-person Medicare Annual Wellness Visits and other preventive visits
• complete and submit required documentation
• coordinate with the Quality Team
• train staff and providers on AWV workflows
• identify workflow barriers
• support system-wide AWV completion and quality initiatives
• provide occasional acute walk-in care when appropriate and needed onsite.
