The Bilingual Emergency Room Navigator role is onsite in Chicago, IL, with a full-time schedule whose hours vary according to patient and operational needs. Compensation is $23 per hour. Humboldt Park Health is a community-based, 200-bed acute care hospital serving the Humboldt Park community. The navigator helps patients engage with ongoing care by completing assessments, connecting them with community resources, and coordinating referrals and appointments. The role also supports behavioral health coordination, patient outreach, and accurate documentation of assessments and interventions.

What you'll do

• Complete assessment tools with patients arriving at the emergency department or ambulatory clinics
• assist with community resources and appointment scheduling
• facilitate referrals for clinically indicated services
• refer patients to primary care for follow-up
• coordinate behavioral health care with primary care providers and mental health providers when appropriate
• encourage patient engagement using calm communication, motivational interviewing, and behavioral activation
• document patient information, assessments, interventions, and encounters
• conduct outreach
• develop referral relationships
• prepare resource materials and reports
• perform administrative duties and maintain department orderliness.
