Dialysis Social Worker

The Dialysis Social Worker role is onsite in New York, NY. Pay is $40–$45 per hour. The role contributes to coordinated care by assessing psychosocial needs, helping patients access benefits and community resources, and serving as a liaison among care providers.

What you'll do

• Conduct psychosocial assessments and develop social work treatment plans compatible with medical care
• understand and explain available Medicare, Medicaid, Blue Cross, and other insurance benefits
• assist patients in obtaining benefits
• participate in multidisciplinary conferences and care planning
• use crisis intervention techniques as appropriate to assessed needs
• refer patients and families to community agencies and assist with access
• coordinate information for transfers or transient visits among the dialysis center, hospitals, nursing home, and other dialysis centers.
