What you'll do

• Serve as the primary contact for assigned patients
• provide individual counseling, case management, treatment planning, referrals, and guidance. Maintain patient records and required reports, lead or co-lead therapy and educational groups, coordinate care with staff, participate in treatment team meetings, and follow confidentiality and HIPAA requirements. The role includes regular rotation on the mobile unit and travel requirements of less than 10%.
