Transitional Services Specialist

The Transitional Services Specialist is an onsite, full-time role in Saint Paul, MN. Compensation is $18–$23 per hour. The Metropolitan Center for Independent Living supports individuals with disabilities in living independently by removing barriers and promoting choice. The specialist coordinates practical services that help people move from licensed facilities or other dwellings into community-based homes, working with consumers and service providers to arrange household items, furniture, and moving support.

What you'll do

• Coordinate transitional-services referrals and moves into community-based living
• coordinate outreach to community agencies
• document consumer contacts in company databases
• order and track household and furniture items
• provide updates and troubleshoot missing items
• pick up and deliver orders from local stores and moving companies
• provide customer service to case managers and consumers. A valid driver’s license, vehicle access, and proof of insurance are required.
