What you'll do

• Travel to new markets to lead health center launches, establish clinical workflows, and onboard initial patient panels
• cover provider vacancies at existing centers
• provide full-spectrum outpatient primary care, including preventive services, chronic disease management, and acute illness treatment
• perform primary-care procedures
• educate and counsel patients
• collaborate with pharmacists, dietitians, care coordinators, and behavioral health partners
• document care in the EHR
• participate in quality initiatives, provider meetings, case reviews, and continuing education
• support community outreach and practice growth.
