Hospice Community Liaison

Job overview

The Hospice Community Liaison role is onsite in Brentwood, CA, with a base salary of $60,000–$80,000 per year plus OTE commission. The role supports Madrona Hospice Care’s referral development and community partnerships. This position develops and maintains ethical referral relationships across the service area, educates healthcare partners about hospice services, and coordinates referral information with intake and clinical teams. It helps connect appropriate patients with hospice evaluation while leaving eligibility and admission decisions to qualified clinical and medical personnel.

What you'll do

• Develop and maintain referral relationships
• conduct in-person visits to healthcare and community partners
• educate referral sources about hospice services and referral procedures
• facilitate referrals to intake and clinical teams
• coordinate referral documentation and communication
• manage territory plans and account pipelines
• maintain accurate activity and referral records
• follow ethical marketing, HIPAA, Medicare, Medicaid/Medi-Cal, and agency compliance requirements.
