The clinician assesses clients’ needs, develops case plans, and provides or coordinates interventions across the continuum of care. The role reviews clinical documentation, supports client safety and rights, and coordinates with agency staff and community resources to help clients access appropriate services.
What you'll do
- Assess client service needs using approved Department of Mental Health assessments
- develop case plans
- provide interventions defined in treatment plans or link clients to appropriate resources
- review clinical documentation and client response to treatment
- maintain accurate and timely records in AVATAR
- participate in treatment team meetings
- preserve client safety and rights
- respond to communications
- provide public information and consultation as needed
- transport consumers in a company vehicle or use a personal vehicle for work duties with mileage reimbursement
- conduct outreach, which may include home visits
- meet the stated productivity requirement of 110 billable hours per month.
