Hospice Social Worker

The Hospice Social Worker role is onsite in Warwick, RI, serving patients and families across Rhode Island. Compensation is up to $45 per hour, depending on license and experience; the role is expected to support up to 20 hours per month. Athena Hospice of Rhode Island provides hospice care throughout the state. The social worker assesses patients’ and families’ psychosocial needs, provides counseling and education, and contributes to individualized care planning. The role helps the interdisciplinary team address social and emotional factors affecting patients and families during terminal illness and end-of-life care.

What you'll do

• Assess patients’ and families’ psychosocial status and patient residences
• evaluate family dynamics, caregiver abilities, communication patterns, and risks, then plan interventions
• counsel patients and caregivers as needed
• educate the team about cultural needs
• participate in individualized care planning and interdisciplinary meetings
• support family planning for funeral, financial, legal, and health care decisions
• participate in quality improvement activities.

Why this role
