Weekend Case Manager

Job overview

The Weekend Case Manager works onsite at a partner hospital in Odenton, MD, on Saturdays and/or Sundays. Pay is $60 per hour, with eligibility for performance-based bonuses. SilverStay works with hospitals, families, and post-acute providers to coordinate safe, timely hospital discharges. The case manager leads weekend discharge coordination, addresses barriers, and works with hospital teams, families, and post-acute providers to support safe transitions. Documentation and use of SilverStay's Complex Case Tool help track progress and support hospital throughput goals.

What you'll do

• Coordinate safe weekend discharges for complex patients
• address barriers in real time
• collaborate with hospital staff, physicians, and families
• use the Complex Case Tool for discharge tracking and accountability
• coordinate post-acute placements with SNFs, ALFs, rehabilitation, and home health providers
• provide weekend discharge leadership
• handle offsite coordination tasks when needed
• document progress, barriers, and outcomes.

Why this role

Requires an RN, LMSW, or LCSW license, BLS/CPR certification, and at least one year of onsite adult inpatient hospital case management and discharge planning experience. Weekend shifts are 8 hours, Saturday and Sunday, 8:30 AM–5:00 PM; initial onsite training and job shadowing are required and paid.
