What you'll do

• Develop and grow referral relationships with physicians, hospitals, skilled nursing facilities, assisted living and senior living providers, case managers, discharge planners, and other healthcare sources
• prospect new referral sources
• maintain existing relationships through field presence and follow-up
• execute sales and marketing plans to grow census and market share
• track activity and pipeline in sales software or CRM
• communicate with internal operations, clinical, and leadership teams
• represent the agency at community events and professional meetings
• maintain knowledge of local market conditions and referral patterns.
