What you'll do

• Complete OASIS documentation
• perform detailed patient assessments and develop patient-specific plans of care
• provide direct home health nursing under physician orders
• document clinical records according to agency policy
• coordinate with team members and referring physicians
• provide treatment notes and other required documentation promptly
• begin discharge planning early and prepare the patient, team, and physician.
