What you'll do

• Provide direct in-person patient care during on-call, weekend, and after-hours shifts.
• Conduct assessments, admissions, routine visits, and death visits.
• Support the Interdisciplinary Group (IDG) through chart reviews and care plan updates.
• Educate families on disease progression and symptom management.
• Maintain accurate clinical documentation and support survey readiness.
