Registered Nurse (RN) Case Manager — PACE Program

Complete Senior Care is a nonprofit Program of All-Inclusive Care for the Elderly (PACE), providing a continuum of services through an interdisciplinary team. The RN case manager assesses participants’ clinical, functional, psychosocial, and home-safety needs and manages their plans of care with the interdisciplinary team. The role tracks outcomes, revises care plans as needed, and supports ongoing participant management, including aide supervision, reassessments, and home visits.
