Case Manager, Family Shelter

The Case Manager, Family Shelter role is onsite in Roxbury, MA, with work at the shelter and in the community. The stated pay is $24–$28 per hour. The case manager orients families to the 24-hour program, assesses needs, and connects residents with appropriate resources. The role focuses on stabilization, future planning, and goal setting while monitoring children’s safety, health, and education needs and maintaining required case documentation.

What you'll do

• Complete family intake procedures
• conduct case management meetings at least weekly, with two to three meetings preferred when needed
• coordinate monitoring of children’s safety, health, and education needs
• refer children under age three for early intervention evaluation and assessment within 30 days of placement
• support and advocate for survivors and families affected by domestic violence, sexual assault, and human trafficking
• help clients identify strengths and employment goals
• submit case notes and reports
• assist Housing Case Managers with monthly rehousing plans
• maintain records in ETO/ASSIT
• maintain confidentiality and professional boundaries
• travel locally as needed.
