This role supports the Mosaic Contractors team in providing a supportive home environment for individuals with intellectual disabilities. The role exists to create a sense of belonging by welcoming an individual with an intellectual disability into your home. The provider supports the individual's daily living skills, medical needs, and community integration to help them lead a life of independence and growth.

What you'll do

• Support daily living skills such as cooking, budgeting, and cleaning
• assist with medication administration and medical appointments
• provide transportation
• maintain documentation
• provide behavioral and physical support as needed.
