It supports continuity of resident care by communicating changes in condition and coordinating schedules with clinical staff and outside providers.

What you'll do

• Organize resident transportation according to appointment priority and needs
• transport residents to and from appointments, programs, facilities, and residences, including after hospital discharge. Report observed changes in resident condition to clinical supervisors, transport medical records for signatures, maintain the vehicle and its mileage and maintenance logs, assist with safe transfers as directed, coordinate with outside providers, and provide hydration according to resident needs.
