• Provide one-on-one support tailored to individual needs
• support personal hygiene, daily living skills, meals, medications, appointments, household tasks, and community activities
• encourage socialization and skill building
• communicate with families and team members
• collaborate with case managers and healthcare professionals
• complete timely documentation.
