What you'll do

• Develop and update individual service plans
• advocate for individuals' preferences and goals
• monitor provider adherence to DDSD Waiver standards, company policies, and individual plans
• conduct regular in-person visits
• coordinate with the individual's team
• maintain records and required reports
• support access to community life, employment-first opportunities, and protection of rights
• promote health, safety, accountability, and independence.
