What you'll do

• Oversee clinical services provided by the interdisciplinary team in homecare and facility settings
• review referrals and patient information
• assign hospice personnel
• evaluate cases through visits, conferences, and record review
• guide clinicians
• conduct supervisory visits
• review diagnoses, prognosis, medications, procedures, and clinical course
• support care planning
• attend case conferences
• conduct quarterly record reviews
• support hiring, orientation, training, and education
• collaborate on service improvement and intake processes.
