Part-Time GPD Veteran Case Manager

Job overview

The Part-Time GPD Veteran Case Manager role is onsite in Durango, CO, at the Durango Community Shelter, with pay of $23–$25 per hour. The role supports homeless veterans enrolled in the Grant Per Diem program. Volunteers of America Colorado develops community programs for people facing critical needs. The case manager assesses eligibility, conducts intakes, coordinates services, and provides case management. Connecting veterans with housing, employment, health, and other resources supports their stability and participation in the program.

What you'll do

• Assess eligibility and conduct intakes
• coordinate services leading to housing
• work with the VA on eligibility, benefits, and support services
• develop life-skills programs and participant activities
• locate volunteer and community service opportunities
• maintain case notes and reports
• conduct weekly veteran house meetings
• provide crisis intervention
• maintain the facility
• drive as required.
