What you'll do

• Assess client housing needs and develop individualized housing plans
• connect clients with affordable housing, rental assistance and community resources
• coordinate with landlords, property managers and housing agencies
• support housing applications, eligibility and documentation
• navigate CityFHEPS, Section 8 and other housing programs
• maintain case notes and client records
• advocate through applications and waitlists
• conduct follow-up visits and check-ins
• collaborate with internal staff and external partners.
