What you'll do

• Assess residents and evaluate changing health needs
• coordinate ancillary health services, treatments, and medication administration
• maintain medication, treatment, and resident service records
• monitor Care Partner performance and infection-control practices
• respond to resident and staff concerns
• train and supervise Care Partners
• coordinate pharmacy orders and therapeutic diets
• participate in service plan conferences
• follow the scheduled on-call rotation.
