Social Work Staff (LSW) - Clinical Case Management

Provides psychosocial assessment, individual and family counseling, care planning, case management, and community education. Works collaboratively with Case Management and the care team to evaluate psychosocial needs and develop discharge plans. Evaluates patients’ needs for continuum of care and effective transition from hospital. Participates in multidisciplinary rounding.
