What you'll do

• Conduct in-home assessments for LT-PCS participants.
• Evaluate client needs for eligibility and care planning.
• Provide Choice Counseling for HCBS programs.
• Educate applicants on Medicaid long-term care.
• Develop person-centered Plans of Care.
• Coordinate services with providers and community organizations.
• Complete monitoring visits and case documentation.
