Managing the care of a defined patient population; developing expected patient outcomes, defining appropriate resource utilization and setting customer satisfaction goals. Serving as a clinical expert, and providing education and consultation services to internal and external groups. Collaborating with other Lee Health Case Managers, Social Workers, Physician and other caregivers to facilitate car
