Community Supports Lead Care Coordinator — Housing

What you'll do

• Conduct outreach, intake assessments, and in-person follow-ups
• coordinate housing navigation, tenancy support, and deposit assistance
• develop housing care plans
• connect members with housing and community resources
• coordinate with landlords, shelters, providers, and community organizations
• maintain accurate documentation
• support program compliance
• and collaborate with interdisciplinary teams. The role spends approximately 65% of its time in the field and requires local travel.
