What you'll do

• Act as liaison for patients, families, and the multidisciplinary clinical team. Assess clinical, emotional, spiritual, psychosocial, financial, and other patient needs
• facilitate resolution and monitor follow-up. Educate or reinforce education about disease, treatments, side effects, adverse reactions, and reportable signs and symptoms. Coordinate discharge planning and help ensure instructions are understood and followed. Develop or coordinate patient education materials, clinical pathways, and quality studies
• participate in multidisciplinary consultations or tumor conferences and document recommendations
• help develop understandable and feasible treatment plans
• support Intake Coordinators
• promote community education and prevention and early-detection screening.
