What you'll do

• Conduct client screenings and intake interviews
• develop and monitor individualized plans
• coordinate medical, psychosocial, and psychiatric services
• facilitate access to transportation and basic needs
• provide crisis support
• conduct street and community outreach
• distribute survival and hygiene supplies
• maintain case records
• coordinate discharge and post-discharge support
• contribute to program evaluation and reporting.
