What you'll do
- Build relationships with older adults in assigned communities
- represent Matter Health
- educate prospective patients about the care model and services
- support outreach, enrollment, scheduling, and ongoing engagement
- conduct in-person outreach, phone calls, and community events
- partner with advanced practice providers, social workers, care navigators, and operations teams
- identify barriers and connect patients with community resources
- document work in Salesforce and Athena
- support patient retention and healthcare access.
