The Clinical Liaison Hospice Inpatient Admissions Registered Nurse (PRN) works onsite at U of M, Southdale, and St. John’s locations in the Saint Paul, MN area. This is an as-needed position, with primary weekend coverage Saturday and Sunday, 8:00 a.m.–5:00 p.m., and some flexibility in scheduled hours. Pay is $45–$50 per hour, based on experience. The role supports AccentCare’s hospice services for patients and families. The nurse identifies patient needs and facilitates transitions into hospice care by assessing patients, coordinating admissions, educating healthcare partners about hospice eligibility, and collaborating with care teams. The role also advocates for patients and families during goals-of-care conversations and supports timely referral coordination.

What you'll do

• Build relationships with physicians, hospital staff, and care teams through in-person collaboration
• serve as a clinical resource for hospice education, eligibility, and appropriateness
• conduct patient assessments and coordinate admissions
• partner with admissions, clinical, and care teams on transitions
• advocate during goals-of-care conversations
• participate in interdisciplinary planning, training, and outreach
• meet response-time, referral-coordination, and communication expectations.
