Alumni Coordinator

What you'll do

• Conduct outreach by calls, texts, and emails for at least one year after discharge
• track interactions and follow-up in Salesforce or a similar CRM
• support Joint Commission aftercare requirements
• facilitate one weekly alumni group with a licensed therapist
• coordinate and attend one weekly H&I commitment
• develop alumni engagement strategies
• review and correct intake contact and emergency information
• build rapport before discharge
• connect alumni with resources
• encourage alumni and family reviews, with a target of at least five per week
• monitor well-being
• and coordinate with admissions and clinical teams on re-entry into care.
