What you'll do

• Coordinate care within the integrated primary care and behavioral health team
• enroll eligible and interested patients in the Circle of Care project with the behavioral health clinician and primary care provider
• conduct clinically appropriate visits and calls
• maintain a patient registry
• arrange higher levels of care and discharge coordination
• coordinate information among care team members
• identify treatment barriers and possible solutions
• maintain community referral relationships
• facilitate therapy and psychiatry referrals
• participate in staff training. LCSWs provide licensure supervision to LMSWs.
