What you'll do

• Provide in-person and telephonic medical case management
• discuss treatment plans and alternatives with physicians
• assess, plan, implement, and evaluate patient progress
• attend provider, home, hospital, and long-term facility visits or discharge planning conferences as applicable
• conduct initial home evaluations
• coordinate durable medical equipment and nursing services
• travel locally up to approximately 60% of the work week/month
• overnight travel may be required
• comply with safety rules.
