What you'll do

• Coordinate and participate in MDS, CAAs, and care plan completion under federal and state regulations
• coordinate assessment schedules and communicate changes
• participate in daily PPS, weekly Medicare, and month-end meetings
• gather and verify assessment information
• monitor completion and signatures
• observe nursing care and review documentation
• assist with chart reviews and audits
• collaborate with nursing and interdisciplinary teams on resident condition changes and care plans.
