MDS Coordinator

Job overview

The MDS Coordinator role is primarily onsite in Springfield, IL, with one remote-work day per week. Compensation is $30–$45 per hour. The position supports facility care planning and coordination across nursing, dietary, social recreation, restorative, rehabilitation, and physician services. The coordinator ensures assessments and care plans are complete and timely, and oversees MDS/PPS documentation and submission. What you'll do

• Assess new admissions and determine health status and level of care
• complete Medicare, quarterly, annual, and significant-change MDS assessments on time
• maintain assessment schedules and reference dates
• complete and update resident care plans within regulatory time frames
• schedule and conduct resident care conferences
• oversee restorative nursing measures being reflected in care plans.
