Registered Nurse Case Manager and Recertification Nurse, Home Health

The Registered Nurse Case Manager and Recertification Nurse role is a field-based position serving Georgetown, Mount Orab, Sardinia, and surrounding areas in Brown County and Southern Ohio. The part-time role requires availability one to two weekdays between 8:00 AM and 5:00 PM, at least four weekly visits, and travel within the service area. Pay starts at $35–$40 per hour based on travel coverage, with mileage reimbursement. Home Care Network is a nurse-owned agency focused on patient care and wellbeing. The nurse coordinates and reassesses home health services, provides nursing care, and documents patient needs to support continued medically necessary care and optimal outcomes. The role also communicates across patients, families, physicians, caregivers, and agency staff and provides field leadership to care teams.

What you'll do

• Conduct client assessments and direct, plan, and initiate appropriate home-care actions
• manage and supervise nursing and aide staff
• coordinate and document patient status and changes with the care team
• perform case management, admissions, recertifications, transitional care, resumptions of care, clinical reviews, and other assessments
• provide backup skilled nursing visits as needed
• submit timely documentation in compliance with applicable guidelines
• travel throughout the assigned service area.
