Registered Nurse — Home Care, PRN

Job overview

The Registered Nurse home-care role is onsite in Beltsville, MD, with travel to clients’ residences. Compensation is not specified. The nurse reports to the Director of Clinical Services and provides care under each client’s plan of care. The RN coordinates and delivers skilled home nursing services, evaluates client needs and progress, and works with families, physicians, supervisors, and other care providers to support continuity of care. The role also documents services and supervises paraprofessional care as assigned.

What you'll do

• Conduct comprehensive health and psychosocial assessments
• monitor client condition and provide skilled nursing care under the care plan
• teach and train clients and caregivers
• coordinate services and referrals
• complete admission, transfer, recertification, resumption-of-care, and discharge documentation
• consult with physicians and supervisors about care-plan changes
• maintain clinical notes and visit tallies
• supervise and evaluate paraprofessional care and LPNs per agency policy
• participate in case conferences and discharge planning.

Why this role
