The role contributes to patient safety and continuity through care aligned with plans overseen by an RN and applicable safety guidelines.

What you'll do

• Provide one-on-one skilled nursing care to pediatric patients in the home
• work with registered nurses and clinical leadership
• follow clear plans of care and CDC safety guidelines
• use provided personal protective equipment. Requirements include a valid Pennsylvania or multistate LPN license, TB test, in-person BLS CPR card, and valid driver’s license
• G-tube, trach, or ventilator experience or willingness to train.
