What you'll do

• Review patient information and determine home-care needs
• assign appropriate clinicians
• review diagnoses, medications, procedures, and clinical courses
• supervise and mentor clinical staff
• conduct performance evaluations and provide feedback
• facilitate training and professional development
• collaborate on patient care plans
• monitor outcomes and adherence to clinical guidelines
• resolve clinical issues
• assist with screening and interviewing personnel
• and perform duties delegated by the Clinical Director.
