Job overview
The Case Manager is an onsite role based in Farmington Hills, MI, supporting individuals with brain, spinal cord, and medically complex injuries. What you'll do
- Provide intake, assessment, crisis intervention, and advocacy.
- Conduct home visits and coordinate care with multidisciplinary teams.
- Develop and monitor individualized care plans.
- Assist with benefits eligibility (Medicaid, Medicare, Social Security).
- Maintain up-to-date case records and progress notes.
